Customised Yoga for BJJ – Intake Form


I’m excited to create a personalised yoga practice tailored to your specific needs—whether it's improving overall mobility, addressing stiffness or pain, or enhancing your BJJ performance.

Here’s how we’ll work together:

  1. Fill out this form with details about your body—tightness, pain, movement restrictions, and what you want to achieve.

  2. I will review your responses and get back to you with a suggested plan and pricing.

  3. If you’d like, we can have a quick video call to go over details and customize your plan, or we can communicate via email/WhatsApp—whichever you prefer.

  4. Once we confirm the plan and payment is processed, I will record your personalized sequence(s) within 10 days and send you the videos to practice on demand.

  5. After you've practiced the sequence(s), we can have a 30-minute follow-up session to go over any questions, modifications, or next steps.
